Home and Community Based Services

The Home and Community Based Services Program is designed to serve people in the community who would otherwise require nursing home care.

Services are individually prescribed and arranged according to the individual needs of the recipient. An individual Plan of Care is developed by the Care Management Team in conjunction with the recipient and the attending physician.

For those individuals who don't qualify for Medicaid, Private Pay Contracts are also available.

Eligibility Requirements (Medicaid):

  • 65 years or older
  • Medicaid eligible
  • Intermediate or skilled level of care
  • Reside in one of the eleven counties or two reservations covered by Area II Agency on Aging.

Available Services:

Adult Day Care: A place for individuals to go during the day that provides both stimulation and socialization.

Dietitian: Nutritionist provides evaluation and monitoring of nutritional status, counseling, therapy, education and research services for the individual.

Grief Counseling: Support for family members to deal with the issues of loss and the death and dying process.

Habitation/Home Modification: Arrangements for installation of devices such as wheelchair ramps and bathtub bars.

Homemaker Services: Help with general household duties such as meal preparation, laundry, cleaning and shopping.

Personal Assistance Services: Provides supervision for health and safety reasons.

Respite Care: Short-term relief for persons normally providing care. Overnight respite care, if available.

Respiratory Therapy: Under direct supervision of a respiratory therapist and/or physician.

Medical Alert Monitoring: A small device worn on the individual that can be triggered by a push of a button to alert significant others to respond in case of illness or accident.

Nutrition/Meals: This includes the availability of someone to come into the home and prepare nutritional meals as needed.

Transportation: Social, religious and non-medical transportation services.

Occupational Physical/Speech Therapy: For maintenance purposes, provided or supervised by licensed therapist.

Psychological Consultation: A licensed Clinical Psychologist will consult with providers and care givers directly involved with the recipient.

Nursing: R.N. and/or L.P.N. provides medically necessary nursing services when not available from a home health agency.

Additional Community Services: Such as home health nursing are arranged as needed.
